Background Checks, Drug Test, and Health Records

We are glad you are here!

Bon Secours Mercy Health (BSMH) does not currently require the COVID vaccine, but vaccines are still highly recommended and encouraged. If you have a CastleBranch account and have received vaccinations or boosters, please upload documentation.

Some clinical agencies MAY REQUIRE students to be vaccinated against COVID prior to clinical placements. Your program’s Clinical Coordinator will have more information about placement requirements. While you can answer "no" to the COVID vaccination in your CastleBranch tracker per BSMH's policies, you may be required to obtain the vaccination or approved exemption (if allowed by the site) prior to clinical placement.

Placing CastleBranch Orders: Clinical students place an order with the vendor CastleBranch, the depository for clinical documentation. CastleBranch reviewers ensure that submitted information meets the program requirements. Verify that all personal information (name, date of birth, SSN, etc.) is entered accurately at the time of order placement or additional fees may be incurred. Students who have been absent for more than three semesters must contact their program administrator for a renewal package code.

Clinical Requirements: Detailed information is provided for each of the requirements. Share the Health Screening and Requirements Overview and Health Clearance documents with your health care provider to determine what blood tests ("titers") are needed to prove immunity and any additional screening that must be completed. The Health Clearance attests that you are up to date with requirements and are cleared for entering the health program and must be uploaded to CastleBranch.

Program Specific Requirements (Community Health Worker, EMT/Paramedic, Medical Imaging, MSN and Physician Assistant students only): Detailed information regarding extra requirements for specific programs.

Some programs require students to place more than one order - follow the ordering instructions carefully.

Accelerated BSN Students ONLY (Summer Semester)

SU-24 students place an order for the appropriate package below:

MT32summerABSN: ONLY for students currently in the state of Ohio OR students who will be able to travel to Ohio to complete fingerprinting.

MT32summerABSNout: ONLY for students located outside the state of Ohio AND who will NOT be able to complete fingerprinting in Ohio.

Toledo FA-24 students will place TWO orders:

  1. Step One: After admission to program, order MS96fpbg (fingerprint/background check)
  2. Step Two: Once results have been cleared by the Nursing Clinical Coordinator, order the clinical requirements clearance/drug test: MS96fall

Toledo FA-24 students will place TWO orders.

  1. After admission to program, order MS98fpbg (fingerprint/background check)
  2. Once results have been cleared by the Nursing Clinical Coordinator, order the clinical requirements clearance/drug test: MS98fall

FA-24 students will place TWO orders:

  1. Step One: After admission/readmission to program, order MT32fpbg (fingerprint/background check)
  2. Step Two: Once results have been cleared by the Nursing Clinical Coordinator, order the appropriate clinical requirements clearance/drug test: MT32fall (new students or returning first year students)

FA-24 students order MT41fall

SP-25 students completing a clinical/practicum will place TWO orders:

  1. Step One: Order MS95spring (background check, drug test and clinical requirements clearance).
  2. Step Two: Once a clinical/practicum site has been approved and finalized, order the appropriate package for fingerprinting clearance:

NOTE: Spring 2025 package codes will not be available until after November 15, 2024.

FA-24 students will place TWO orders:

  2. Step Two: Once a clinical/practicum site has been approved and finalized, order the appropriate package for fingerprinting clearance:

  2. Step Two: Once results have been cleared by the Radiologic Technology Clinical Coordinator, order MS97fall (clinical requirements clearance/drug test)

FA-24 students completing a practicum will place TWO orders:

Step One: Order the appropriate fingerprinting package based on the location of practicum site. The practicum site will be based on two things:

  1. Where student holds an RN license, and
  2. Where the College has a Clinical Affiliation Agreement. Contact the Program Director to discuss your options before ordering this package. DO NOT order until practicum site has been finalized:

Step Two: Order YE76fall (background check, drug test and clinical requirements clearance).

SP-25 students order YD48spring

NOTE: Spring 2025 package codes will not be available until after November 15, 2024.

Graduate Programs

SP-25 students completing a clinical/practicum will place TWO orders:

  1. Order EY61spring (background check, drug test and clinical requirements clearance).
  2. Once a clinical/practicum site has been approved and finalized, order the appropriate package for fingerprinting clearance:

SP-25 students place an order for the appropriate package below:

YC15spring: ONLY for students currently in the state of Ohio OR students who will be able to travel to Ohio to complete fingerprinting.

YC15springout: ONLY for students located outside the state of Ohio AND who will NOT be able to complete fingerprinting in Ohio.

IMPORTANT: Additional order(s) may need to be placed throughout the program resulting in additional fees. DO NOT place additional orders until directed to do so by your Program Administrator.

NOTE: Spring 2025 package codes will not be available until after November 15, 2024.

FA-24 students completing a practicum will place TWO orders:
